Florida LMHC licensure application checklist.
This Florida LMHC licensure application checklist begins after supervised experience is complete. Florida reviews a separate licensure application, supporting documents, examination result, required courses, fee, and background screening. Finishing the experience does not automatically convert a Registered Mental Health Counselor Intern registration into an LMHC license.

Short answer
Before applying for full Florida LMHC licensure by examination, confirm that the Board can verify your qualifying graduate education, supervised experience, passing NCMHCE score, required Florida courses, electronic background screening, application answers, and current fee. The Florida Board’s LMHC page is the controlling procedural starting point.
A completed internship is not a license. Continue using the legal status shown in Florida’s official records until the Board issues the LMHC license. An application receipt, file number, deficiency-free portal, or approval email should not be treated as a license number unless the official record shows the license has been issued.

Confirm that the requirements are complete
Begin with the full Florida LMHC licensure requirements. The current Board path includes qualifying education, two years of post-master’s supervised experience, at least 1,500 hours of face-to-face psychotherapy, at least 100 supervision hours in no fewer than 100 weeks, a passing NCMHCE result, required courses, electronic fingerprinting, and a complete licensure application.
Review dates as carefully as totals. Under section 491.0045, Florida Statutes, Florida experience obtained before intern registration does not count. The Board also requires approval of the Qualified Supervisor before experience under that supervisor counts.
If your education, supervisor, setting, hours, or examination record is uncertain, obtain an individual Board determination before submitting based on your own interpretation. A checklist can organize the file but cannot decide whether a particular record qualifies.
Graduate education documents
For licensure by examination, the Board’s current supporting-document list includes an official transcript from the qualifying master’s or doctoral program. Review the current education instructions for the 60-semester-hour or 80-quarter-hour framework and the programmatic-accreditation rules that apply to the degree date and route.
The Board page lists a practicum letter for non-CACREP applicants. It describes an exception for specified CACREP mental health counseling programs. Do not omit a practicum record merely because the word “counseling” appears in the program title. Confirm whether the Board’s stated exception applies to your exact program and accreditation record.
Official transcripts normally must come through an approved secure transcript service or directly from the institution in the manner the Board accepts. A student copy, screenshot, or document uploaded by the applicant may not satisfy the official-transcript requirement.
Have the numbers ready before you apply
Most of the delay on this checklist comes from supervised-experience records that were never kept in one place. The Florida Hours Tracker is a free Google Sheet that holds the running totals as you go, so the figures on your application match your log.
Supervised-experience verification
Each Qualified Supervisor should complete the Board’s Verification of Clinical Experience Form. The Board currently limits the form to one Qualified Supervisor, so applicants with more than one supervisor should expect a separate form for each person.
Compare every verification with the identical supervision logs maintained by the intern and supervisor. Confirm the beginning and ending dates, registration number, supervisor license number, weeks, supervision hours, psychotherapy hours, signatures, and submission route.
Current Rule 64B4-2.0025 says a Qualified Supervisor must submit a complete and accurate verification within seven days after supervision concludes. The Board’s registered-intern webpage separately says the form is not required until the applicant is ready to submit the full licensure application. Because those instructions appear inconsistent, ask the Board which timing applies to your record.

NCMHCE records
Florida currently requires the NCMHCE for the LMHC path. A passing result must reach the Board through the accepted official process. The Board says passing scores are generally forwarded in approximately four to six weeks, while NBCC’s score-report guidance says state-licensure scores are automatically sent to the state board approximately four weeks after testing.
If you tested for another jurisdiction or the score does not appear, follow the current vendor and Board instructions for an official transfer. Keep your candidate ID, registration confirmation, score document, and any transfer receipt.

Required Florida courses
The current Board page lists an eight-hour Florida Laws and Rules course and a three-hour HIV/AIDS course from a Board-approved provider shown in CE Broker. It also lists an HIV/AIDS affidavit pathway where applicable. Verify the live language before relying on the affidavit rather than a course.
The Board lists a two-hour Domestic Violence course to be completed within six months after licensure. Because this requirement falls after issuance, place it on a dated post-licensure checklist instead of delaying the application unless the Board instructs otherwise.
Keep the completion certificate for each course. A purchase receipt or course-registration email does not establish completion. Confirm that the provider and course meet the Board’s current CE Broker requirements.
Electronic background screening
Beginning July 1, 2025, Florida expanded fingerprint-based background screening to Chapter 491 applicants. The Department of Health’s current background-screening instructions direct applicants to an approved Livescan provider participating in the Clearinghouse.
Use the profession-specific ORI displayed on the live Department page, obtain the Transaction Control Number, and complete the required privacy-policy affirmation. Do not copy an ORI from an old article or appointment receipt without checking the current official page.
Fingerprint-vendor charges are separate from the Board’s licensure fee and can vary. Keep the vendor receipt and control number, but do not upload sensitive fingerprint or criminal-history information to an informal service or email address.
Application, disclosures, and fee
Use the Board’s current online application or the current PDF. Read every criminal, disciplinary, health, and history question carefully. When an affirmative answer requires a personal statement or supporting record, follow the application’s exact instructions rather than submitting a brief unexplained “yes.”
Names should match across the application, identification, transcripts, exam record, supervisor forms, and background screening. If a name changed, provide the documentation requested by the Board. A mismatch can prevent records from being associated with the correct file.
As of August 23, 2026, the Board lists a $100 nonrefundable application fee, $75 initial licensure fee, and $5 unlicensed-activity fee, for a $180 total. Confirm the live fee table immediately before paying.
Submit and track the file
After the application is submitted, the Board issues a file number for tracking. The Board explicitly says this is not the license number. Use the MQA Online Services Portal to link the application, monitor status, review open deficiencies, and upload eligible documents.
The Board page also lists email, fax, and mail routes for supporting documents. Some records, especially official transcripts and examination scores, have their own required delivery routes. Do not assume every item can be uploaded by the applicant.
Keep a dated submission index with the document, sender, delivery method, confirmation number, and date it appeared in the portal. This is practical organization, not a Board-created requirement.
Deficiency letters and approval letters
The Board describes two common review outcomes. A deficiency letter identifies missing items required to complete the file. An approval letter includes the license number and additional information about the newly issued license.
Respond to each deficiency using the route stated in the notice. Do not resend the entire file unless requested, since duplicate records can make tracking harder. If a requested item was already sent, provide the delivery confirmation and ask the Board how it should be matched.
Under section 456.013(1)(a), Florida Statutes, and the Board’s current page, an incomplete application expires one year after initial filing. Treat that as a firm file-management deadline and respond early enough to resolve follow-up questions.
After the license is issued
Verify the license through the official License Verification search. Use LMHC status only after issuance. Save the approval letter and a copy of the official record.
Complete the two-hour Domestic Violence course within six months after licensure if it was not already completed in a manner the Board accepts. Review renewal, continuing-education, fingerprint-retention, display, employer, payer, malpractice-insurance, and business-record updates that apply to your work.
Update professional profiles carefully. Do not imply that the Board endorsed your practice, reviewed this guide, or approved a specialty beyond the license it issued.

Practical checklist, not a substitute for the current Board application:

- Intern registration and every supervisor approval retained.
- Two years, 1,500 psychotherapy hours, 100 supervision hours, and at least 100 weeks reconciled.
- Verification of Clinical Experience completed for every Qualified Supervisor.
- Passing NCMHCE score sent through the official process.
- Eight-hour Laws and Rules certificate retained.
- Three-hour HIV/AIDS certificate or currently accepted affidavit completed.
- Electronic fingerprinting completed under current instructions.
- Application answers reviewed for consistency and required explanations.
- Current $180 Board fee confirmed and paid.
- File number saved and application linked to the portal.
- Deficiency responses tracked before the one-year expiration.
- Two-hour Domestic Violence deadline placed on the post-license calendar.
Frequently asked questions
Should I wait for my NCMHCE score to appear before applying?
The Board permits records to arrive separately, but an incomplete file cannot be approved. Decide based on the current application instructions, your one-year file deadline, and whether all other requirements are complete.
Does my supervisor submit my personal supervision log?
The Board’s application materials emphasize the Verification of Clinical Experience Form. Maintain the identical official supervision logs and provide them if requested. Do not substitute an informal spreadsheet for a required Board form.
What if I had more than one Qualified Supervisor?
Use a separate verification form for each supervisor and reconcile the dates and totals so hours are not duplicated.
Can I call myself an LMHC after receiving an approval email?
Rely on the issued license number and official verification record. If the approval message and public record differ, contact the Board before using the title.
